Each address here belongs to one person. They use it to request and receive their own health records.

If you got a request from one of these addresses

• The patient sent it, or it was sent on their instruction. It is their own request for their own records, under their right of access.
• Send the records, and any questions about the request, to the same address. The patient receives everything sent there.
• If you need to verify the patient's identity, email the address. They will reply from it.
